Last year, Honeywell and Strata Manufacturing (Strata), the advanced manufacturing facility wholly owned by Mubadala Investment Company PJSC (Mubadala), entered a partnership to produce made in the UAE N95 masks.

In an interview with Omnia Health Magazine, Roman Poludnev, General Manager of Safety and Productivity Solutions Middle East, Russia, Turkey and Africa, Honeywell, said that the partnership helped fulfil the rapid spike in demand for Personal Protective Equipment (PPE) caused due to the pandemic. While hospitals had certain PPE reserves, due to the lack of clarity and guidance on use, PPE consumption, especially among healthcare workers, reportedly increased up to 17 times compared to usual consumption. Also, due to challenges created by the pause in airline operations, even those who were able to produce PPE were not able to ship them out.

“Honeywell has always had a local strategy of fulfilment,” said Poludnev. “We have been in the Middle East for over 60 years. Because of the nature of our engagement with customers, we always prefer to serve them from existing locations. For PPE and masks, we have several factories worldwide but did not have one at the start of the pandemic in the Middle East. So, due to the surge in demand, logistical challenges, and restrictions, we collaborated with Strata to build this facility.”

He highlighted that the facility, located in Al Ain, was built in record time – from the decision to build it to production to release took just five weeks. Moreover, the facility can produce more than 90,000 masks per day.

From an efficiency perspective, the locally produced N95 and FFP2 masks go a long way in protecting healthcare workers from contracting the virus. The masks can reportedly filter 95 per cent of large and small solid and liquid particles. Masks that are not certified to the N95 standard may not provide this level of protection against the inhalation of smaller airborne particles. These N95 masks also provide a leak-tight seal that helps minimise toxic particles from entering the mask and are designed to provide comfort and protection. Unlike medical or surgical masks that provide a loose fit, these masks offer a proper fit.

When asked about how quality standards are maintained, Poludnev said that the facility uses the same technology and machinery part of Honeywell’s blueprint globally. “However, during the pandemic, sourcing raw materials such as polypropylene became a challenge. That’s where Strata stepped in and helped us solve the problem locally. We also went through thorough checks with our global laboratories to ensure that it meets the required standards. What we produce in UAE is absolutely identical to the masks we produce around the world,” he emphasised.

Compared to last year, the situation has stabilised, and logistics has significantly improved, but there are still some challenges to be addressed, the General Manager stressed. For example, at the start of the pandemic, when there was an acute shortage of N95 and FFP2 masks, many sub-standard and counterfeit masks made their way into the market and continue to be in circulation. “These masks offer no protection. However, Honeywell and Strata have addressed this issue by introducing an application that allows scanning a barcode on the mask packaging and by downloading it users can validate whether the mask is original or not,” he added.

A company statement highlighted that before the opening of this mask operation, N95 masks were typically imported to the UAE. Therefore, the partnership has also transformed the UAE into an international exporter of N95 masks to help respond to the spread of COVID-19 around the world.

Enhancing productivity

While COVID-19 put the spotlight on critical worker safety, the issue of productivity also became a concern. Poludnev shared that with the collaboration between Honeywell’s Auto Identification business and the Nursecall system, the company developed several tools for nurse communication. The primary purpose of these is to remove unnecessary actions taken by the nurse. For instance, instead of constantly checking up on the patient face to face, the tool allows the nurse to speak to the patient, even if they are at home. The solution connects a patient at home to the doctor or nurse in the hospital with the help of hardware and software solutions.

The company’s solutions have also helped in the manual removal of patient admission by replacing it with Optical Character Recognition (OCR) scanners at hospital sites. Honeywell has provided several hospitals in the UAE with these scanners to scan Emirates ID and link it to the healthcare database at the hospital without any manual paperwork. Furthermore, the company has developed a mobile Nursecall system that doesn’t require infrastructure within the hospital and automates data entry into hospital information systems.

One of the other most important lessons learned from the pandemic, said Poludnev, was that the education of customers is a crucial area. In the coming months, he said the company is aiming to engage with more healthcare partners, who will make customers more aware of their tools and solutions that exist in the market. Also, the firm will continue to expand locally through different partnerships.

He concluded: “We will continue working with experts in the healthcare industry and apply our software analytics tools and hardware to offer solutions that are targeted towards productivity improvements of healthcare workers, as well as their safety.”

The first robotic heart surgeries were performed in the late 1990s and have come a long way since then. Today, several generations of robots are available and best suited for patients who have problems with the heart that are better managed without a sternotomy – including those who would potentially have complications if their sternum was split in half. Over the years, these robots have evolved to incorporate different subspecialties and offer better visualisation and magnification, more instrumentation, and more dexterity provided by the robotic arms.

In an interview with Omnia Health Magazine, Dr. Husam H. Balkhy, Professor of Surgery and Director, Robotic and Minimally Invasive Cardiac Surgery, University of Chicago Medicine, explained that the majority of the patients that get robotic surgery are the ones that have isolated valve or coronary problems. It can also be applied to several other cardiac problems such as arrhythmia problems, benign tumours of the heart, congenital defects in the heart such as septal defects. Also, pericardial disease can be treated with robotic surgery.

Dr. Balkhy has been with UChicago Medicine for over eight years and is one of the most experienced robotic heart surgeons in the world, with more than 2,000 robotic heart surgeries under his belt.

He said: “The Holy Grail of what we would call robotic surgery is that the robot would do the surgery on their own, but we have not come anywhere near that yet. So, it's a misnomer when we call it robotic surgery because it's not the robot doing the surgery, but the doctor carrying out the surgery with the facilitating technology of the robotic system.”

However, the doctor stressed that the robot facilitates technology that allows the surgeon to do the procedure without making a big cut. “There's no limit to what the robotic approach can achieve. It requires experience and knowledge of how the system works and can lend itself to many different problems in the heart, and in other specialities as well, such as urology, gynecology, or general surgery,” he added.

Path to recovery

In traditional open-heart surgery, the majority of the patients are restricted in their movements and ability to lift heavy weights or do physical activities for at least three months until their breastbone heals. “In the traditional approach, we take a saw and cut the breastbone up the middle and spread the ribs apart. Then, when the surgery is over, we put that back together, and it has to heal. This is termed as a controlled fracture – a fracture that we create to get access to the heart, which then has to heal and, therefore, requires limitations in physical activity,” he explained.

But when asked about what life is like for patients after undergoing robotic surgery, he said that no physical restriction is put on them after the surgery. The patients will be in the hospital for a couple of days and recover at home for a week or two. Most people will be able to go back to their full-time jobs within two to three weeks.

UChicago Medicine’s robotic cardiac surgery programme, highlighted Dr. Balkhy, receives patients from across the U.S., as well as from around the world. These patients turn to robotic surgery thanks to its benefits, such as fewer complications compared to conventional surgery and a quicker recovery rate, leading to a shorter hospital stay and an earlier return to full activity.

He shared the story of an emergency room (ER) doctor from Portland, Oregon, who had planned a trip around the world with his family for a year. He was in the ER one day, had chest pain and ended up being diagnosed with coronary blockage. He was recommended to undergo open-heart surgery. When he asked whether it could be done without cracking his chest open, he was directed to UChicago Medicine’s robotic cardiac surgery programme.

“His trip was supposed to start about three weeks from the time he was diagnosed. We were able to do the surgery, and within a week after his operation, he was on a plane to Greece to start his trip with his two daughters,” he shared.

There was also a patient in his 70s for whom a robotic approach was beneficial, as he had two prior surgeries on his mitral valve, both of which had failed. He needed to have a third operation, which the UChicago Medicine team was able to do robotically. Doing a third conventional surgery would have been detrimental to his health. “He was a singer in the choir at his church, and his mitral valve disease affected his ability to project because he was just too short of breath. After the surgery, he was back in the choir within a month,” he added.

At the pandemic’s peak, surgeries were shut down to curb the spread of COVID-19 for about four to five weeks. However, one of the benefits of robotic surgery is that it allows patients to be discharged from the hospital much quicker, reducing the risk of contracting the infection.

Dr. Balkhy emphasised: “We felt that doing surgery with the robot is very appropriate in this time of wanting to limit people's time in the hospital and their susceptibility to becoming infected, but also in providing access to patients on a quicker basis. In addition, minimally invasive surgery is more suited for a pandemic situation as it allows more patients to have access to hospital beds.”

The future of robotic surgeries, he said, will see more specialities adopting the robotic platform and more technology becoming available to facilitate different types of procedures.

“In the future, more patients will have access to this type of technology, which can prevent a lot of morbidity and potential complications. In any scenario, heart surgery is complex and trying to minimise the pain and the disability afterwards is crucial.

“The new robots that are coming out in the next five years would hopefully bring the cost of the technology down and make it more available to people around the world,” he concluded.

Message on World Heart Day

On the occasion of World Heart Day, Dr. Balkhy stressed that patients need to be their own advocates and do research to try and understand if their condition can be treated with a less invasive approach and where they could potentially find that.

“The number one message I would like to share is that people need to emphasise prevention and do all the healthy things – take care of their diabetes levels, not smoke, live a healthy lifestyle and exercise – that prevent heart disease. Unfortunately, however, a lot of the heart disease that we treat has nothing to do with any of those things. If you have a leaky valve or a hole in the heart, that can’t be prevented. But if you need to have surgery, then my advice would be to do some research and look into your options,” he said.

Psoriasis is a common, chronic, inflammatory, and proliferative condition of the skin, associated with systemic manifestations in many organ systems. According to Dr Dhanya Rajkumar, psoriasis is a noncommunicable, disfiguring and disabling disease that significantly negatively impacts the quality of life for those with the disease.

The most common characteristic of the skin rash is red, scaly, sharply demarcated lesions that are seen particularly over the extensor surfaces and scalp. Psoriasis can also affect the nails and joints. According to the International Association of Psoriasis Federations, approximately 125 million individuals across the globe are affected by psoriasis.

According to Dr Rajkumar, a specialist dermatologist at Medcare Hospital Sharjah, special consideration should be given to lifestyle factors and behaviours that contribute to the overall health and treatment responsiveness. These include weight management and quitting smoking. In addition, the use of thick, non-fragranced moisturisers daily is recommended in conjunction with additional baths, which can be infused with colloidal oatmeal, Epsom salts or coal tar to help alleviate inflammation, irritation, dryness, and redness.

For reasons yet to be known, the use of UV light is said to improve the remission of psoriasis lesions. Thus, while most individuals avoid sunbathing to decrease the risk of developing skin cancer, those who have psoriasis may benefit from it.

There is no known cause of psoriasis; however, there are several lifestyle factors to consider to reduce its appearance and spread.

“It is suggested that excessive alcohol consumption is associated with moderate to severe psoriasis and should therefore be avoided,” Dr Rajkumar explains. “Smoking also causes an increased risk of developing psoriasis.”

Also, any infection that affects the immune system can trigger a certain type of psoriasis, which is why there can often be a flare-up following an ear infection, bronchitis, tonsillitis, or even respiratory infection.

Dr Rajkumar also explains that increased levels of depression, anxiety and worry often results in flare-ups of the disease, leading to moderate or sometimes even severe psoriasis. Even though sunlight is suggested to be beneficial to individuals with psoriasis, in some cases, psoriasis may be provoked by sunlight, so if a person experiences an aggravated case of psoriasis following sun exposure, future minimised exposure is advised.

Treatment for psoriasis

There are various treatment modalities for psoriasis, depending on the type and severity of the disease. According to Dr Rajkumar, mild plaque psoriasis without psoriatic arthritis can be treated with various topical treatments such as coal tar, potent topical corticosteroids, vitamin D analogues, followed by phototherapy as a second line of treatment.

In moderate to severe plaque psoriasis treatment, phototherapy is the first line of treatment, followed by oral medications such as immunosuppressants and apremilast.

“In case patients do not respond to the above treatment, the next step is biologicals, which can have a great promise for individuals with moderate to severe psoriasis, and they represent a significant step forward in the development of psoriasis medications that will improve the quality of life of patients,” Dr Rajkumar concludes.

“Biologics are systemic drugs that target only specific parts of the immune system. Biologics used in psoriasis target cells and proteins that cause inflammation, the rapid growth of skin cells, and damage to the joint tissue, e.g., TNF alpha, interleukins 12, 17,23. Therefore, biologics selectively targets these proteins, thereby reducing the psoriatic symptoms.”

It is commonly known that older medical inpatients are only moderately active in the hospital. In Augmented exercise in hospital improves physical performance and reduces negative post hospitalisation events: a randomised controlled trial, published by BMC Geriatrics, patients reportedly walk an average of 600 steps per day, which translates to 12 minutes of walking; 49 per cent of elderly patients are on bedrest or simply transition from bed to chair, and less than 19 per cent of patients walk in hospital hallways. The study suggests that patients who walked more had a shorter hospital stay, with a 50 per cent increase in step count being associated with a 6 per cent reduction in hospital stay, while those with poor physical performance on admission were the least active in hospital. Following a hospital stay, these frailer patients are at a higher risk of functional impairment.

The trial was carried out to compare the impact of an augmented prescribed exercise programme versus the standard care on physical performance, quality of life, and healthcare utilisation in frail elderly medical patients admitted to the hospital. The method was based on a parallel single-blinded randomised controlled trial. Older medical inpatients with an expected stay of fewer than three days and who need help or assistance to walk were assigned to the intervention or control group within two days of arrival. Both groups got half-hour guided exercises twice a day, Monday through Friday, from a staff physiotherapist until release.

The intervention group completed personalised strengthening and balancing exercises, while the control group stretched and relaxed. The major outcome metric was the length of stay. Readmissions within three months, as well as physical performance (Short Physical Performance Battery) and quality of life (EuroQOL-5D-5 L) at discharge, were all assessed secondary variables.

Time-to-event analysis was used to compare lengths of stay, and regression models were used to compare physical performance, quality of life, adverse events (falls, fatalities), and negative events (prolonged hospitalisation, institutionalisation).

Data from 190 individuals (aged 80 ± 7.5 years) were assessed out of the 199 patients assigned. At the start, the groups were comparable. In an intention-to-treat analysis, there was no difference in length of stay between groups (HR 1.09 (95 per cent CI, 0.77–1.56) p = 0.6).

Physical performance was better in the intervention group at discharge (difference 0.88 (95 per cent CI, 0.20–1.57) p = 0.01), but lost at follow-up (difference 0.45 (95 per cent CI, − 0.43 – 1.33) p = 0.3). An improvement in quality of life was detected at follow-up in the intervention group (difference 0.28 (95 per cent  CI, 0.9–0.47) p = 0.004). Overall, fewer negative events occurred in the intervention group (OR 0.46 (95 per cent CI 0.23–0.92) p = 0.03).

To conclude, improvements in physical performance, quality of life, and the occurrence of fewer negative events show that this intervention is beneficial to fragile medical inpatients. However, its impact on the length of stay is not defined.

Leading Austrian 3D printing service provider Addion GmbH, which specializes in customized 3D printed medical model production, has selected Stratasys’ J750™ Digital Anatomy™ 3D printer to produce never-before-seen surgical models for ophthalmology. 

Using the full color multi-material 3D printer and collaborating closely with medical training solution provider and manufacturer Eyecre.at. GmbH, Now, Addion and Eyecre.at. are able to develop and construct artificial eye models with a level of realism that was previously unachievable.

This technology provides pharmaceutical companies and providers with true-to-life models that enable improved product testing and surgical training.

To date, ophthalmology has lacked high-quality artificial eye models that mimic the exact anatomy of a human eye including color, haptics and complexity. Addion’s 3D printed eye models allow pharmaceutical companies to develop, advance, and demonstrate medical devices, while medical practitioners can perform surgical training on more standard eye procedures. 

By its very nature, the production of 3D eye models is generally a lengthy and complex process. Due to the eye’s watery and ultra-soft tissue, it can be difficult to easily recreate ophthalmologic anatomy using existing medical scanning technology, which means that models are developed from anatomical drawings. The models created using previous 3D printing technologies lacked true-to-life accuracy due to limitations in material options and color capabilities.

“For us, the possibilities with the Digital Anatomy Printer are endless, and despite the incredible results we’ve enjoyed to date, we are still just scratching the surface in terms of the technology’s potential,” says Hechenberger. “There are hardly any comparable alternatives for eye model production in our industry, particularly in the complex recreation of soft tissue with realistic colors and the variety of materials.

“For example, we produced an eye model where the cornea (the white part of the eye) can be detached and reattached. The skin layer on the model is a tenth of a millimeter and is detached with very small fine instruments, implanted, and then reattached. This level of detail was previously unimaginable.”  

In addition to the Stratasys 3D printer, Addion is using Stratasys’ GrabCAD Print Software to optimize the design-to-print workflow.

“Our early experiences using GrabCAD have been wholly positive,” says Hechenberger. “The software ensures an efficient workflow from data to printing and will hopefully prove invaluable as our experience with the Digital Anatomy Printer grows.”

Looking forward, Addion GmbH is already planning to produce other anatomical models in order to make full use of Stratasys’ unique material offerings like BoneMatrix™ for simulating bone structures, and TissueMatrix for vascular structures.

Wolters Kluwer, Health has announced a collaboration with Ecaresoft that will combine the leading drug screening solution Medi-Span® Clinical with Cirrus electronic health record (EHR) and Revenue Cycle Management (RCM) solutions to offer a fully integrated NPHIES compliant solution to healthcare providers.

The move will enable both companies to deliver on a shared global mission to ensure that everyone has access to the best care possible by offering best-in-class technologies to healthcare providers in the region.

Medi-Span Clinical is used by hospitals and healthcare systems in 30 countries to screen patient data against drug information within the EHR, alerting clinicians of patient safety risks from allergies, drug interactions, and potentially inappropriate dosing.
